The Fox Valley Carpenter’s Place mission is to rebuild lives of those that could face homelessness and the near homeless using a holistic case management approach using web based software housed on a server at Northern Illinois University. This case management program, titled the Life Recovery Plan, is used to perform an intake on a client where core issues are identified in 14 areas of their lives.
Client goals are set with client’s input and acceptance, are tracked and relevant case notes documented. The case manager holds the client accountable for the goals that are set by providing them with action plans and steps. |

The value of having the program web based is the ability to store client information, with client authorization, to help synchronize the resources being applied to a particular client, and to facilitate collaborative case management. The technology is very user friendly with drop down screens available to assist in answering questions during the intake. A data base of available community resources is also available on a drop down screen to assist the case manager in identifying the best resource for the client to achieve the goals set.
Fox Valley Carpenter’s Place is serving the near-homeless population in the Aurora and Fox Valley region. Being near-homeless can be caused by many circumstances such as recent job loss, going through divorce proceedings, medical bills may have inundated you, or perhaps legal issues have caused financial hardship in your life.
